The muscles of the posterior compartment are separated by intermuscular septae, thus defining a superficial group and a deep group of muscles. After the muscles of the superficial group have been identified, they will be cut and reflected to expose the muscles that lie deep to them.

First, cut the gastrocnemius muscle just proximal to the point where the heads converge. This leaves intact the muscular branches of the tibial nerve that enter the deep surface of the muscle. Reflect the gastrocnemius muscle inferiorly and superiorly .
