In the next two steps the gluteus maximus muscle will be cut and reflected to expose the structures that lie deep to it. Care must be taken to avoid damaging the underlying structures.
Starting at the inferior border of gluteus maximus muscle, cut along the medial border of the muscle and detach it from the posterior surfaces of the ilium, sacrum, coccyx, and sacrotuberous ligament.After the muscle is freed from the medial attachments, reflect the gluteus maximus muscle laterally. During the reflection of the muscle, identify the inferior gluteal nerve and vessels and superior gluteal vessels on its deep surface. Segments of the gluteus maximus muscle, which surround the vessels and nerves, may be cut from the muscle to preserve these vessels and nerves for later study.
