Examine the short muscles of the little finger that form the hypothenar eminence. Medially, identify the abductor digiti minimi muscle. It arises from the pisiform bone and inserts into the medial side of the proximal phalanx of the little finger. Laterally, locate the flexor digiti minimi brevis muscle. It arises from the hook of the hamate and flexor retinaculum and inserts into the base of the proximal phalanx of the little finger. Cut both of these muscles near their insertion and reflect them proximally to expose the underlying opponens digiti minimi muscle. It arises from the hook of the hamate and flexor retinaculum and insert into the entire length of the fifth metacarpal bone.
